At the recent LLL of NJ Area Conference I attended a session on Breastfeeding Multiples.  Two of the three presenters used a double sling set-up and demonstrated quite effortlessly how they put one over each shoulder and then slipped the babies in.  I am sure it takes a bit of finesse but they made it look so simple!
