Family physicians must retake boards every seven years to retain board
certification.  (You can certainly practice without board certification,
though.)
In my state, you must reapply for licensing as an MD every 3 years, and must
have 150 hours of CME credit -- although any subject is fine, and no one
really checks that you learned anything.
The family practice board exam is the same for new grads and recertifiers.
I don't recall any question on breastfeeding when I took the test in 2001.
