In utero, maternal blood that manages to sneak past the placenta gets
into the baby's bloodstream where it can provoke hemolysis (breakdown of
red blood cells) of the baby's blood. Hemoglobin is recycled into
bilirubin, and depending on the degree of hemolysis, baby may become
jaundiced.

In milk, maternal blood gets into the baby's stomach, where it is
digested. No harm done (some babies do get an upset stomach if there's
LOTS of maternal blood).
