Hi everyone,
I need your expert advice. I have searched the archives and all of my
resources but also want to hear from those of you who work with these
little ones on a regular basis. I provide support to a small hospital who
send out our babies born < 35 weeks and sick babies so we don't have a lot
of experience working with tiny weak ones.  We have had a lot of small  35
weekers lately who are very reluctant feeders in the first few days. Most
of these babies unfortunately are born to moms who have a difficult time
bringing in the milk supply ( hypertensive, gestational diabetic, etc
etc .. you know the story ). We have gotten to the point only recently were
the nursing staff are OK with waiting a bit in the first 24 hours for the
term infant before hitting the panic button and giving formula supps if
baby is not feeding well. We are trying to establish some guidelines for
the near term little ones and wonder what the best practice evidence based
approaches are. Of course we encourage skin to skin, expression of and
supps of colostrum if baby is too weak to do much at the breast but what if
mom gets little or no colostrum in the first 24 hours. What is your
approach to feeding the SGA and /or 35-37 weeker in the first 24-72 hours.
Can anyone point me in the right direction to find some guidelines,
protocols and  specifics on this.
