I would say the tongur-tie could explain a lot of this...

If severely restricting tongue movement, can cause the "humping" and the
lack of organized suck/grooving...often feels like thrusting.  Baby gets
mom's let-down, then fusses and pulls off repeatedly because unable to
extract milk on own.  Mom's description of pinched nipplke and vasospasm can
certainly be the cause of pain during and after feedings. I doubt oversupply
is the issue based on the amount of wt gain since birth.  Only thing I
cannot explain with the tongue-tie is the reflux, which may be allergy, and
contributing to fussiness.

I would find someone to clip the tongue, start Vit B6 150 mg/day, heat to
nipples immediately after feedings.  When all else is resolved (or if
fussiness worsens in the meantime) remove all dairy from mom's diet.
