Can the pediatrician provide evidence to back up that belief?

I'm preparing to discuss this with a pediatrician who believes the diagnosis can be made by diet history alonw, more specifically, that if the mother reports that the child is six months old or more and exclusively breastfed, the child is by definition malnourished.  So I need to know if there is something I have missed.
