Hmmm, this is difficult.  There could be many causes.... What are the 
mother's nipples like??  Did the mother receive pethidine during late 
labour?  And what techniques have you already tried to get the baby 
to latch?  Have you considered a shield if the milk is already coming in?

Meanwhile, hopefully the mother is expressing and pumping her milk 
and feeding the EBM (150ml/kg/day).
