Here's something that is not a current case, but I'd like your opinions
nonetheless.

Awhile back I received a call from a pediatrician who wanted my opinion.
In her practice there was a premie born to a mother who was nursing a
toddler. The new baby was expected to be in hospital for 3-4 weeks.
The nursing staff told the mother she must wash her nipples (with
handwashing soap) before nursing the premie since the toddler's germs
were a concern. I advised against the soap--but had difficulty offering
an opinion (new one for me ;-) ) What would you do/say?
