The studies that the AAP statement are based on do not have well defined
definitions of what is breastfeeding i.e. they include exclusively breastfed
babies along with those who may have breastfed a few times in their lives in
the same sample group. The authors of the statement certainly didn't assess
the comparison of infants who were allowed to have unrestricted access to
mom's breasts and used no pacifiers to those who had some formula, some
breastfeeding and some pacifier use. What I think the studies were not
looking at were the risk of the impact of formula use; rather they were
studying the benefits of breastfeeding without quantifying it.

There are other problems with the statement, but please read ILCA's response
to get an idea of what they are.
