I am way behind on my Lactnet posts, so forgive my response to something
written more than a week ago

The survey at the site below is not a new survey of formula cost and is not
current. It is a copy of a 1998 BSC handout.  I have a pad of those I
purchased from them.  I mistakenly thought this site had new current costs
of formula, and copied it to use as an up to date handout.  When I compared
the two to see how much costs had gone up, I found out the truth.  It is a
direct copy of the handout minus the copyright information and date.
