It depends on your state licensing laws, but in the situation you
described, a nurse or registered dietitian should be able to recommend
herbs or drugs as long as you are not prescribing them.  Everywhere I've
worked (in long term care, not L & D or NICU's) I've made
recommendations to physicians when I determine that changes in
medicines, supplements, or even diet orders are needed.  It is then up
to the MD to take my recommendation and write orders or not.  I would
check with your state licensing board for specifics on
recommendations/orders and scope of practice guidelines.
