I was referring to the aggressive removal of milk specifically from the
blocked segment using hand expression. This is done (here) by usually using
the thumb(s)  to "strip" the offending duct(s), moving from the distal area
of the blockage toward the nipple. Often the material blocking the ducts can
be removed (usually a cheesy or clotted milk material in appearance)
followed by the freer expression of milk as the blockage is moved. I am not
keen on this method when the breast is inflamed, either from a blockage or
mastitis, as I believe it may incur even more damage to the already inflamed
tissue. Also its very painful for the mother - I much prefer to get the baby
on there and do the work in combination with some GENTLE massage.
