Re smoking and SIDS.  I once had an overnight guest who smoked--though not in my house--and I had to was the sheets the next day due to the tobacco smell in them.  Not a good place for an infant!

Re shallow palate, active baby.  When I was an intern, we saw a young mom and her baby.  The mom was concerned that he wasn't getting enough milk, but breastfeeding seemed to be going well by all other indicators, except that his weight gain was slow.  The mom kept saying how he was always moving and didn't sleep.  I had her walk me through 24 hours of his sleeping, nursing, and activity, and he was sleeping 7 hours per day and in constant motion when he was awake, which we could easily see.  The kid was just burning calories super fast.  She did end up supplementing with formula and he started gaining more normally.  This mom was overweight, definitely not a super-athlete.  No advice, just noting that it does happen.  Solutions will vary.
