Hi Laura,

What is urine output like? Poos? Still some weight gain?? Bright and alert? 
How much supplementing with other than EBM? How much dummy/pacifier/thumb? I 
also thought hormonal like Christina. What about 
ovulation/menstruation/pregnancy? Does bub have blocked nose, post nasal 
drip, ear problems, sore throat, etc?

Just a few more ideas to think about?

Could be classic early fast flow, good supply, bub doesn't have to work hard 
for letdown, supply stabilises and bub now can't be bothered suckling for 
longer to get letdown. Lots of relaxation to get letdown happening, perhaps 
breast/nipple stimulation before latching baby.

Your description also suggests bub could be very efficient suckling for just 
a few minutes, flow then slows and then bub fusses, wants to swap sides? 
Does she do this?

Pumping quantity is not a good indication as to what bub is getting - this 
mum would obviously be stressing and so a letdown may not come easily.

Good on Dr suggesting increase number of feeds.

Looking after 3 little ones is hard work, is she looking after herself 
getting help?? Also her confidence has been shattered with what she's been 
through with #2 bub.

Now she has baby #3. Cute little boy who was 8 lb at birth, gained well 
enough by 2 months of age. Now at 4 months he is only 10 lb 7 oz. Dr says to 
increase breastfeeding & return to get wt check in one week. Mom states she 
is nursing all the time now, baby nurses for 5 minutes, she is not feeling a 
let down, can only pump 2 oz when baby misses a feeding (she had a little 
milk stored so baby rec'd this by bottle). I saw her at a group meeting & 
baby goes to breast for a few minutes, then starts pulling at her nipple, 
sucks several times, pulls nipple again, repeats a few times & then releases 
nipple & fusses. Baby then sucked on pacifier and went to sleep. She is 
pumping several times a day after breastfeeding but doesn't get enough for 
supplementing. We talked about her coming for a full consultation, using a 
rental pump & SNS, but she did not do either at this time. Her pediatrician 
is supportive of breastfeeding, so may offer her a prescription at next 
week's visit.
