Depends on how hypoplastic the breasts are. There is very little hope 
for severe hypoplasia, but if it is mild domperidone and goat's rue 
along with very intenstive stimulation have been helpful for moms in my 
practice. One particular mom who had significant difficulties with her 
first child made a full milk supply with the above interventions by 6 
weeks postpartum. She breastfed frequently, used a Lact-Aid 
(supplementer device) with donor milk as needed for the first 6 weeks. 
The milk was individually donated, but the donors took the same blood 
tests done by HMBANA and the blood test results were vetted by the 
baby's pediatrician. She had this donor milk on hand before the birth 
and started using some on day 2 or 3, when baby's diaper output started 
to become insufficient. (This mom was motivated).

Petok and Huggins (I think) found that a proportion of mothers in their 
study with hypoplastic breasts developed sufficient milk supplies by 6 
weeks. The study was published in Clinical Issues in Human Lactation, I 
think in 2000 or 2001. Very useful work.
