Dear Cindi,
       Your description of the mother's pain sounds very much like ductal
thrush.  Even though she has no history of recent antibiotic use, she had nipple
cracks the first couple days after birth.  Skin is one of the natural barriers
to infection, but any break in the integrity of the skin can allow yeast or
bacteria to enter and gain a foothold.  It can take some time for the infection
to grow to the point of pain, but I do think that ductal yeast sounds like
this mom's problem.
