My off-the-cuff reaction to this is, why is this a problem?  It sounds as if
it has only happened twice, and, in any event, vomiting the breastmilk and
not the formula is probably just coincidental.

Even if not, the baby is probably retaining more than it looks.  My
firstborn routinely vomited large amounts of breastmilk with every feeding
but nevertheless had wonderful, even spectacular weight gain, so was she was
obviously getting enough.

Perhaps there is more to this than I'm seeing, but I am wondering if there
really is a problem there.

Is the breastmilk going in faster than the formula perhaps (I'm sure it
tastes much better - is baby taking more of it in a shorter time period?)
