Hello,

First of all, I'd like to say that last night I attended a very enthusiastic and 
inspiring round table discussion hosted by the Yale New Haven Hospital 
breastfeeding council.  For anyone that was present or working in CT., it was 
a thrill to be there.  I think we've made good progress in achieving consistent 
care throughout the community and it was a privelage to be present.  I am 
looking forward to keeping up the momentum at the next meeting.  It was so 
great to be amongst so many professionals whom share the same goal.  One 
of those common goals being ensuring good breastfeeding care in the pediatric 
practice, being that is a constant with all mom and baby dyads.  It was just 
another reminder as to why I love being in the lactation field!

I am an RN, currently working in private pediatric practice, as well as giving 
breastfeeding support in the home via VNA services.  I have qualified to sit for 
the IBCLC exam in 09' and it couldn't come soon enough.  LOL.  Anyhow, I am 
anxious to incorporate more structured bfing support, expert and evidence 
based into pediatric practice.  I would like to phase out my role as an office 
nurse and start really focusing more on our breastfeeding population and non-
bfing population.  I would like to start a mother's support group, offer prenatal 
bfing teaching, etc.  I would like to see patients in the office and possibly in 
the home supported thru my practice.  Have any of you done this prior to 
certification?  If so, any ideas on coding, reimbursement.  More specifically, 
can a breastfeeding specialist do home visits out of a private pediatric 
practice and get reimbursed as an RN, as I do thru the VNA?  I feel very 
confident in my experience giving bfing support, ethically, do you feel I can 
start to offer care while waiting to take boards?  I want honest opinions.

To make a long story short, my current practice which I have been with for 
years, is closing and dispursing due to retirement, etc.  So I will need to come 
up with a proposal to another practice, one of which I may have my foot in 
the door with.  However, I know I will need to sell myself and breastfeeding as 
an utmost priority for them.  Of course, the dr.'s will want to know they will 
get paid for additional services and time.  I don't think anyone has it in their' 
budget to expand services in this economy without reimbursement.  I know I 
will also need to come up with a fee.  Can anyone give insight to how much 
lc's are being paid in pediatric practices.  Of course, I am not certified yet, 
but would like to give them an idea of where I would like to end up.  

Also, any thoughts on how to start a mother's group??  Do you do weekly, 
monthly?  Do you schedule specific topics of interest, ie. going back to work 
and pumping.  If you have guest speakers or special topics, would you 
consider charging a small fee and offering refreshments, etc.  I have lots of 
ideas, however, don't really know how to implement them.  How do you 
advertise.  I know I will need to present my ideas to the docs and feel like I'm 
not sure how to condense everything into one.  

Most importantly, what are the most important topics for a proposal?  I know I 
am covering a lot for all you wise ones...can anyone lend me a tip or two.
