The situation with tongue tie is not even JUST the baby's tongue - it's also
the mother's breast tissue, innate milk ejection reflex and other factors
not yet known...

I've seen horrible tongue ties with excellent milk transfer - mothers
breasts were like "silly putty" easily drawn in and molded to whatever
issues were in the mouth (I often see high palates with tongue ties). 

I've also seen excellent milk transfer no matter what the baby did if mom
had a great milk ejection reflex...

So no wonder MD's who ONLY look at baby's mouth see that some "do fine"
breastfeeding and others don't!
