Sharron Humenick, RN, PhD and Pamela Hill, RN, PhD published what I consider
classic studies on engorgement in Journal of Human Lactation.  They
described engorgement using a rating scale, elucidated 4 major patterns of
engorgement, and used degree of engorgement as a risk-predictive value for
lactation difficulties.  Additionally, they observed a finding which I have
seen hold true over the years in my own practice:  Previous bfg experience
of the mother is a more critical variable than parity in predicting
engorgement.  I have used their information on a weekly if not daily basis
since I first read it.  I recommend these 2 papers to the person who was
looking for engorgement protocols.  Before you develope the protocol, you
have to have the theoretical basis.  It is not all that easy to find
research on engorgement. Despite it being such a common phenomenon, it is
not well studied.
