Judy,
If I understand what you describe, baby gets ebm, vomits, then takes
formula and doesn't vomit.  Before questioning the milk, I would
suggest looking at the events.  Is the nature of the milk the only
variable involved?  Does it follow this sequence each time?  Has she
ever given the formula first?  Does baby vomit then?  Is there any
chance it is the first thing in baby's tummy that is being
"refunded" rather than what that first thing is?  If so, perhaps
baby needs to be burped before feeding.  You mentioned the
breastmilk had been stored in the fridge.  Has the same thing
happened with freshly expressed breast milk?
