Hi, Carol.
       People make more or less scar tissue in response to any cut in the
skin.  Some have hypertrophic scarring (I think that's the name) and others go
all the way to keloids.  Perhaps this mother had more than average scarring in
the inner tissue of the nipple where the piercing took place, and that scarring
prevented milk from leaving the breast easily?  If this has happened, then
the breast would involute since the failure to adequately remove milk would tell
the breast to stop making it.
