This summer we started using iFormBuilder and their iForm app.  You don't
need a wifi connection in order to do data collection - if you don't have a
connection, it saves completed surveys on the iPad until you have a
connection at which time you just sync it and it uploads whatever has been
collected.  If you have a connection, it uploads a survey immediately after
it is completed.  We are using it for surveys that are conducted as
interviews.  I find it works well for that.  The survey interface looks
more "form-like" than "survey-like" in that all the questions are on a
continuous page rather than separate pages, like you encounter in online
surveys.  I think this works nicely when an interviewer is collecting the
data, but I'm not sure if it is the most user-friendly interface if you're
wanting your respondents to enter their own responses.  I'm still learning
my way through its capabilities on the back-end where you create the
survey, but so far it's been working well for us.
