Hello David!

I am glad you are interested in becoming involved with the SHA.  The plague
passed me over until yesterday and today.  You may send your survey form to
me at the Salt Lake City address below.  I will be in touch with you
off-list regarding involvement in the society.

Please let this also serve as an invitation to anyone else out there who
