This was a very gradual process, generally from the Renaissance period
to the 17th century.  Some old customs lasted into the 19th century, for
example, the use of the tenor clef in orchestral parts for trombones.

I think that the balcony usually gives a better sound than the orchestra
floor, but it depends a lot on the particular hall.  If possible, it's a
good idea to try out various locations until you find one that sounds best
to you.
