Jaroslav Vogel: "Leos Janacek", original published 1962, English
translation edition: Orbis Publishing, London,1981, ISBN 0 85613 0451.
I bought my (hard cover) copy in 1986 but as it is (or was) a pretty
standard Janacek reference, I imagine it's still in print, possibly even
in paperback.  It's an absorbing book and gives many valuable insights.
I strongly recommend it.

Coincidentally, I've just picked up a copy of the old Karel Ancerl version
of the Glagolitic Mass which still sounds great.  It's coupled with two
orchestral pieces by Kabelac: Hamlet Improvisation and Mystery of Time.
Apart from his 5th symphony, I'd not heard anything else by him.  Does
anyone have any other recommendations?
