Well, one thing is to use breast compression to get more high fat milk into
the baby.  If that doesn't work, expressed milk with lactase drops added
could do the trick.

Foreign protein in the milk might do it. Eliminate dairy products from the
mother's diet first, but if the formula was milk based lactose free, it
hardly seems likely the baby is reacting to that.
