A couple of years ago I had a similar situation. I was led way off track
because this was baby #5 and the family had a terrible hx of allergies and
all kinds of illnesses. Mom was feeding and feeding and pumping and baby
wasn't making much headway. At about our fourth visit, baby suddenly showed
good weight gain, and I asked mom what was different about that week. She
mentioned a day that she had suddenly cramped and bled and passed tissue, and
after that her milk supply was much more copious! A quick review of her
delivery noted that the OB had applied pressure on the placenta during stage
three....... need I say more? Case solved.

I now have a mother, G2P2, first time bfg, whom I am again wondering about.
 At 4 days, baby was very fussy, passing gas, not stooling. Ped diagnosed
colic (!!!) and prescribed a suppository (!!!), then sent her to me.  Mother
did not feel milk came in until day 5, and was unsure even then.  BW 6-7,  4
days 5-11 (peds offc), day 5  5-10.5 (my office), day 10  5-10.2 (my office).
 The weight has stayed the same from day 5 to 10, even though mom has been
feeding 12 times a day and baby is stooling 4-6 times, some big, some small.
 Test weight of intake at days 5 and 10 both showed about 1 oz, though mother
felt they were not charateristic of baby's best feeds.......

Mom reports that she bled heavily (~hemorrhaged) after birth and was given
"shots" for it, though she had been on IV pitocin already-- no transfusion.
 She reports continuing to bleed strongly, filling 8-9 long pads one-half
full in 24 hrs. Mentioned that blood "pools" during the night, pours out when
she gets up.  Mom is doing a lot, has been cautioned to slow down. ALSO--
menstrual cycles were always irregular, had been put on progesterone to get
her eggs to release, wherein she immediately became pg with #1. Mother
started gaining excessive wt(120 lbs) after first child and was eventually dx
with pituitary problem causing over production of androgen.  Was put on
clomid, conceived this second baby quickly again. MD hoping that pregnancy
may have effect of correcting problem.

OB office has been unconcerned in the bleeding and unresponsive to my call
thus far; I am hoping to have her rule out retained placental fragment, and
also possibly do a prolactin level to ascertain if there is a hormone
problem.  I would be very interested in anyone else's assessment of this
situation, and other suggested avenues of investigation.  In the meantime, I
have mom supplementing 3 oz/day, pumping after as many feeds as possible, and
taking fenugreek 3 caps TID.
