Cindy-
In my area, I am finding that most of the insurance co.s will cover breast
pump rental when the baby is hospitalized (prematurity, surgery, etc.)  The
pump is considered "durable medical equipment".
The form I give moms to submit lists the rental rates and the purchase price.
 With most companies, they will do whichever is most economical, e.i. rent if
it will cost less than buy.
  With one HMO, I contract directly.  All others must pay me and be
reimbursed by their insurers, although I will give them the appropriate
form.Follow-up is up to them.  If you need info re coding, ins. forms, etc,
