I am wondering if those of you based in the hospital setting are using a consent form (similar to the form to use donor breast milk)for formula supplementation if the mother doesn't want to use donor milk?  We are in the final stages of being able to offer donor milk to not only our NICU patients, but to newborns on the post partum unit who have risk factors.  Had mentioned the formula consent and our director really liked that idea.  If you would be willing to share a copy, I would appreciate it.
