I received so many positive replies that I uploaded the manufacturer's mark tables to Lulu.com this afternoon.  I have ordered my proof copy, which should be here within a week.

I suggest waiting until I send word that I have read the proof copy.   While I think everything is correct, I can proofread better on the printed copy.
