I really think this is a decision best left to the user. Why don't the mother's decide what works best for them? It seems that people have varying results with this approach.
Try without seems the typical first option (using appropriately sized flanges is key IMO),,,and then let the mothers know that assuming they are using appropriately sized shields if they continue to experience discomfort they can try with some lanolin and/or maybe reduce the pump tension.

I will only speak from personal experience that it felt HUGELY better to pump 
with lubricated flanges. I suggest it to moms and recommend they do so if it 
seems to help them
