Wow ! Reading this confirms the need of home visits . As a public health Nurse ,breastfeeding in the computerized charting within my organization only includes  assessing  how infant is receiving nutrition ,not the latch tool or proper documentation unless I type a narrative . I wanted to become a LC to talk the language in my documentation and best serve clients at home while visiting them for c/s wound or infant visit. I began thinking about the obstacles mentioned ,and if a home referral could be created for this matter- home bound is home bound and if the reasoning for a  referral could be newborn assessment / home safety / wgt check certainly the MD could include breastfeeding . I am planning on improving the home support of breastfeeding within my organization when I complete my LC . As well as reaching out to provide these services as a volunteer for those with out health coverage.However this should be a concern for all to receive Lactation Home Visits that meet the criteria related to obstacles r/t family dynamics .Funding for this would be a great asset but tackling this would probably best be initiated to those with medical benefits and homecare visits and then educating Registered Nurses . I know it's only a partial solution but a start/ the lactation consultant may be able to do a joint visit with the nurse much like a wound care specialist does  or nutritionist ,physical therapist etc. I'm in California - these are the possibilities that look possible in my environment. Sorry I posted directly to Ruth but am interested to know if this is already occuring in any hospitals or if homecare nurses like myself provide this service with an LC
