Wow! Rachel, I've heard you loud and clear each time. I cannot control these parents and am not likely to get paid for the help I've given (other than with experience). I didn't expect a verbal spanking but will seriously consider all you have said. I posted here to validate what I knew despite the fact that the Dr. was not more alarmed. I am accustomed to having to advocate for moms who are being urged to give formula and here is this Dr. not raising the alarm. 
I urged her to see an IBCLC or other lactation specialist from the beginning. Nearest IBCLC is 2 hours away. Very few I could find in the directory. Judy mentioned a home birth baby being different and I think that may be part of the puzzle. 
I have urged the mother all along to work on increasing her milk with several techniques and to supplement the baby with the list of choices she has and in the amounts for catchup. She has reported nursing at least every 1-2 hours except for a 3-4 hour gap at night. She was wearing her baby and not really putting him down much at all. He nursed both sides, usually for about 10 minutes. I suggested she use compressions. She uses a cup to supplement. She was not pumping enough and I urged her to increase frequency all along. 
Here I am defending myself even now. 
The midwife helped her find donor milk which they started 48 hours ago. Tonight, they report a third poop and he's gained 2 oz. in the same amount of time. Haven't suggested at breast supplementer bc I cannot be there physically. 
There. I have been quite tired when I've posted and apologize for missing any details. I have appreciated all of the collective wisdom I've received both on list and off.
