Thanks, Catherine.  "Supporting Sucking Skills" was the first thing I
checked out to get a better perspective in this. The tongue stays in the
mouth when at rest and the tongue thickness is WNL. The tongue is just wider
than most, I know that sounds weird.  If I can get a pic, I will. What I
think is happening is that baby's wide tongue isn't fully forming a seal
when cupping the breast (you can even see "excess" tongue sort of flapping
at the sides instead of cupping the breast when you pull the lower lip out
to take a peak).  So baby compensates by using his jaws/lips to hold on
tightly, thrush compressing mom's nipple. I'll be seeing mom tomorrow and
we'll definitely be working on finding a more optimal position. I am
thinking lap straddle, baby led, side kneeling over reclined mom, etc,
etc...we'll play around a bit to see what seems to work best.
