Water therapy in nice warm tub, both naked :-)
Pick up baby when asleep,  Maybe?
Use a <gasp> shield as a transition object?
Had one very successful case, use shield with SNS tucked under, went on to 
not using anything in a few days.  Baby was 7 or 8 wks of bottle feeding br 
milk.
