Hello Colleagues!

I am an evaluator working on an exhibition evaluation. The project has an extremely short turn around time and, rather than spend time re-inventing the wheel to design an 'original' data collection form, I am wondering if someone might be willing to share a data collection form they found useful and give me permission to adapt it for my use (which I am also willing so share)!

Due to budget constraints, data will be collected by volunteers and staff at the museum, once I've trained them in what to do. Data will be collected over 9 days, most likely using observations, interviews and a short check list. Data will be used to make minor modifications in the exhibition. Information is sought from three age groups -- age 6 and under, age 7-11 years and age 12 and over.

Some questions to be asked include but are not limited to:

1. Do visitors understand how to use or interact with exhibit components? If not, what

type of instruction or assistance is required?

2. Do select exhibits (that will be identified) promote individual or group activity? Between adults and children? Children and children?

3. What did visitors like most about the exhibition?

4. What did visitors like least about the exhibition?

5. What would visitors like to see more of or learn more about?

Using the logic model of evaluation, I am, of course, developing indicators for each question above, including both quantitative and qualitative items such as the following:

1. number of people who used the exhibits, as intended (under age 6, 7-11 years, 12 years and over)
2. number of people who did not use exhibits as intended (under age 6, 7-11 years, 12 years and over)
3. suggested change to better allow visitors to interact and use activities?
4. what did people like the most about the exhibition?
5. what did people like least?
6. what would people like to learn more about or see more of?
7. what message did visitors take away from the puppet theatre?
8. number of visitors actually searching for animals in mural vs. number just pushing buttons?

In the spirit of networking, I would appreciate any forms you are willing to share! Thank you!

Terrie
