Hi Cheryl, I forgot to say I have a good "intake" or assessment form, I have
shared with many others, contact me if you wish to try it. At my first visit
with the mom, I do a targeted history and physical. I also have a pretty
postcard with our names, phone no., and all the things we can help with in
the BF Center, about 5 or 6 topics like "concerns about milk supply." This
would be even better if it was a magnet. If there are community resources
like LLL or WIC (most of my clients participate in this U.S. low income
nutrition program for women, infants, and children), be sure to provide
contact info for these.
