I'm working with a mom of a two week old...c-section after 16 hrs
of labor...epidural at hour 10..on drip, then c-s.  Post birth, mother
given morphine.  Really out of it...didn't even see baby until 4 hrs
later.  Formula given because baby had blood sugar drop...bw 9-2.
First time at breast was 8 hrs later.  Screaming refusal at breast.
Floor nurse gave mom nipple shields...and told to pump.  Mom was never told
what to do with colostrum..so she dumped it (!).  Exit wt 8-11.
Mom still pumping giving bm in bottle and kinda trying breast.  Averages 5-6 oz
each time with double attachments.
Baby sucks well, but pulls off screaming and choking.  Hugh amounts of spit up soon
after the breast.  Out-put is all good.  Baby indicates hunger approx. every
3 hrs.  Any feeds at breast after baby settles are brief and baby zonks out.
Mom has alot of food allergies..and has them controlled with elimination all
her adult life.
My question...I've never heard of OMER with the use of a nipple shield.  Can
it be so forceful that it even gushes using a ns?
Suggested pumping a bit prior to feed to min. forceful OMER. Also asked her to
contact LC (LC did not give nipple shields) and ask her about how soon mom can
discontinue use.
Any further suggestions will always be useful.
