At Minnetrista we have tried a variety of formats for teacher
evaluation...it has ranged from "circle 1-5 for quality of xyz" to open
ended questions about the various activities and/or registration
processes.
We've tried mailing evaluations, having evaluations mailed back to us,
on-site evaluations, etc.
The best method I have found thus far is to leave things open ended,
short, and on-site. We give teachers evaluation forms as they begin
their tour. They're on clipboards that have pencils attached. While not
all teachers prefer to fill them out, we do have a very successful
return rate, around 80% or so, compared to when teachers were asked to
mail them back and we got about 3 every semester... We ask only about 8
questions. The first five simply list the activity and then leave lines
underneath for the teacher to write whatever he/she wants. It looks like
this:

That's it! It's amazing what kind of feedback you get. They comment on
the actual tools used, the presenter(s) style and the activity itself.
They often make the connection between what we're doing with the kids
and the state standards that correlate.
The last three questions are all regarding the teacher packet, a
component of what we provide for those who register for our school tour.
Of course, the last line does stipulate that you're free to make any
other comments, suggestions, etc. on the back side of the page...most
choose not to.
You should also talk to Patricia LaBounty at the Union Pacific Railroad
Museum. She is the Outreach Coordinator there and is super passionate
about using good tools for evaluation, I am sure she'd be of great help
to you.

We plan to begin an evaluation of our field trip program. The intended
impact of our field trip program is that students have an increased
understanding of the process of science and of grade level science
content.
In the first phase of evaluation (this fall), we will be focused on
process rather than outcomes in our evaluation. During this time, we
will try to understand teachers' experiences before and during an onsite
visit. I am looking for existing instruments (surveys, interview
protocols, etc.) that might be mined and customized for our immediate
and future purposes.
Our initial study (this fall) will be guided by the questions:
To what extent are teachers satisfied with on-site experiences at our
science center?
How can science center experiences be improved?
Our primary method of data collection will be an online survey completed
by teachers after their field trip visit.
I know the world of field trip evaluation is vast and varied. Any and
all insights, comments, and instruments are very much appreciated!
