I think Lisa Mandell is on to something:  she asks about the mom who *does* have consistent and adequate milk removal, right from the start, but whose supply doesn't seem to boost until weeks (or even months) later.
 
I have heard Kay Hoover speak about gestational ovarian theca lutein cysts (her article about it is at http://jhl.sagepub.com/cgi/content/abstract/18/3/264).  She said that the mothers *must* continue to use the pump for the several weeks it takes for their testosterone to go down ... even when they are only getting drops.  She isn't quite sure why, but it appears to be critical to the process.  So, as Lisa suggests and Kay suspects, mom has to keep those prolactin receptors "happy campers" with lots of stimulation, esp. in the early weeks, if there is to be a pay-off in supply later (once the milk-inhibiting factor is discovered and dealt with).
