Hello Ladies,

I have a client with what look to be large blind milk ducts on her  
areolae...about 15-20 on each...
they are swollen but not leaking...

each one is about the size of the tip of a pencil if you broke off  
just the sharpened part
they are sort of cone shaped...

She has not had a breast reduction, this is a first baby...

She did not experience breast enlargement during pregnancy, but these  
protuberances became more pronounced...

She had an uncomplicated vaginal delivery...no edema,
but onset wasn't until the 6th day...

She does not have visible veining and her breasts are flaccid,
but they are symmetrical and otherwise normal looking- for a non- 
lactating mother that is....

Drops appear on the tip of the nipple as she attempts to put baby to  
breast...no dripping...
She is ten days postpartum.

The baby was supplemented immediately from birth.
And at this point is getting about 10 oz of formula a day.

She does have the beginning of nipple abrasions at the tip of her  
nipples,
but she doesn't have yellow crusting...or any pinkness...

  Her nipple pain is so acute she is guarded as she brings the baby  
to breast
because she can hardly bare the slightest brush of his hands anywhere  
on her areolae...
that is her pain does not seemed to be confined to the abraded tips...

there are no cracks or abrasions at the base or on the sides of the  
nipple....

Her pain does not subside as the nursing progresses...her nipples  
throb after nursing...

So I'm thinking maybe she has infected *Montgomery glands*....and  
considering recommending antibiotics...

I am proceeding as usual, working on building her supply as I would  
with any mom,
but I am wondering if the presence of so many of these blind ducts,
might be a risk factor for insufficient supply...

I hate to make any suppositions...but it does look as if
these *Montgomery glands* are full fledged milk  ducts that
are converging on the nipple but just not reaching it...

Any information or advice would be welcomed.
