The Gold Standard for the program of support you desire is, in my opinion, 
the model created and fine-tuned by Paul Meier at Rush Hospital in Chiacago. 
  Probably buckets of stuff about it in the Archives.  I know she also has a 
modest fund-raising program, selling materials for use in supporting Moms, 
so I'm willing to bet a Google search will bring up lots of information, 
too.  Go hear her speak if you ever get a chance.
