On an Anecdotal level only...
I had a friend who had recurrent plugs and mastitis in her left breast although she was exclusively breastfeeding and baby had constant access to the breast. Upon deliberate evaluation of all aspects of her life (trying to determine the cause) she finally discovered that she was blocking milk flow with her sleeping position. She chronically slept with her left arm tucked under her left breast. Once she made a deliberate and consious effort to change this sleeping habit the plugs and mastitis resolved.
Perhaps this physician could ask her significant other if she tends to sleep with one arm under her breasts?
It may be worth investigating...
