I had a mother recently like that. I could actually feel what felt 
like "pitting edema" on and around her areolas. She was extremely engorged 
and I was unable to get anything out with a pump. I tried hand expression 
which brought out a couple of drops but that was it. I had never seen 
anything like it. Long story short it resolved and she went on to nurse 
fine. I had her try cabbage leaves, leaning her breasts into a pan of warm 
water, pumping, etc., and I'm not too sure what worked but she was 
successful. Sometiems, I think we just ahve to wait it out. In this 
case,that's all that worked.
