Marla, we don't use the steam bags for the nipples because the ones we
use are marketed as a one time use, disposable nipple. However, we do
use the steam bags for moms who are pumping to steralize their pump
parts once a day. I have also had them steam the bottles after washing
with warm soapy water if they don't use the disposable bottles provided
to store milk in and I then encourage them to use the other bottles to
ensure what they are pumping in is not contaminated. 
     As far as the nipple itself in the NICU I look more at the flow of
the nipple more so than the shape. I used to say the nuk nipple needed
to be used for all breast babies but now after reading about the
premmies ability to nipple and observing what they do, I think that flow
is more important. The red premmie nipple is what most people think they
need but if you watch how they manage the milk it gives you a lot of
clues. Is milk pouring out their mouth on each side?  Do they desat or
brady with a strong flow? Do they start coughing or gagging? All of
these are not only clues about the baby's ability to start nipple feeds
but also their ability to manage flow. I have had babies who do all of
the above with a premmie nipple and a nuk nipple but when given a
straight nipple they manage very well. 
     Much of the work Paula Meier has done would be a great place to
look for info on breastfeeding in the NICU. I don't have my articles
with me but I can look for specific references later if you would like.
