Diflucan (fluconazole) is commonly used in newborns and is given even
to medically fragile babies to prevent yeast infections. See this
study in Pediatrics by Kicklighter et al. (2001), Fluconazole for
prophylaxis against candidal rectal colonization in the very low
birthweight infant :
http://pediatrics.aappublications.org/cgi/content/full/107/2/293
The dose given directly to the tiny, premature, medically fragile
infants in this study was 6 mg/kg/day, for 21 days. Hale lists the
theoretic infant dose via breastmilk at 0.34 mg/kg/day. So a baby
who receives a portion of its mother's diflucan via breastmilk is
getting way less than the standard neonatal/pediatric dose.
There is a great deal of info in Hale about diflucan including 11
references. He rates it "L2 - Safer", and it is classed by the AAP
as a "maternal medication usually compatible with breastfeeding."
