I apologize I haven't read the whole msg chain, so someone may have already mentioned these things.

Did the ped diagnose the lactose intolerance from a stool sample? If I remember correct (can't check it out right now), breastfed babies don't fully digest all of the lactose they get, but some of it will remain in the stools. 

The baby's symptoms began after the mom's ab-course, right? Has the mom tried giving the baby some probiotics? They could help balancing the normal breastfed baby's gut flora.

Another possibility could be allergy. I know babies whose allergy started showing after either baby's own or mom's ab-course. But this baby doesn't have other symptoms like rash?
