While this does look like an infected montgomery gland, it is best to 
refer the woman to her physician for diagnosis and treatment. It's an 
important part of the IBCLC standard of practice to know when and how to 
refer to and communicate with physicians to see that our clients' 
medical needs are met while we help them with their breastfeeding concerns.
Our role in this sort of situation is to help the mom with 
breastfeeding, latch, assess the feeding to see if anything the baby or 
mom is doing could have caused the problem and if so help them do things 
differently, and encourage the mom to see her doctor. If she were a 
client, it would be your responsibility to document this lesion and 
communicate your findings clearly to mom's physician.
I'd include the photograph in your report to the physician or midwife, 
whoever is providing mom's maternity and postpartum care.
