The most common reason I hear moms given for not breastfeeding for
48 hours is concern over the anesthesia.  See Hale's Clinical
Conditions book (which I like to call the "reverse MMM") under
"anesthesia".  I always ask, "Why does a patient wake up after
anesthesia?"  The obvious answer is that enough of the med has
gotten "out of her system" to allow her to wake.  If it's gone, or
diluted enough or whatever to allow her to be awake, then it should
be "gone" enough for her to be able to nurse!  I don't have the book
handy, but I seem to remember the general guideline is that if mom
is awake enough to handle baby, it should be OK to nurse.
I wonder where the "magic" 48 hours got started.  It's the same
number given by radiologists for pumping and dumping even when the
substance given does not require mom waiting that long (according to
those who know both the med AND breastfeding). The radiologists are
just looking at the literature from the manufacturer which is always
ultra conservative in order to cover their own you-know-whats.
